The Registered Nurse will provide skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Responsibilities include patient assessments, medication administration, treatment planning, and collaborating with an interdisciplinary team. The role requires strong communication and collaboration skills with patients, providers, and team members.

Responsibilities:
- Provide skilled nursing care.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Monitor cardiac rhythms and manage dysrhythmias.
- Respond to patient emergencies.
- Perform peripheral line care and management.
- Administer respiratory support, including nasal cannula and oxygen masks.
- Adhere to isolation precautions.
- Float to other units as required.
Required Skills & Certifications:
- Active Registered Nurse license in New York state.
- ACLS and BLS certifications.
- EPIC charting experience.
- Minimum 1 year of experience.

Special Considerations:
- Must be available for some holidays.
- Floating to other units is required.
Scheduling:
- 4x12 hour day shifts.
- Every other weekend required.
