The LCSW-Social Worker for Case Management provides psychosocial support to patients with complex needs by collaborating with healthcare teams and community agencies.
They conduct assessments, offer crisis intervention, facilitate referrals, and develop safe discharge plans to ensure seamless continuum of care.
Offering: $3,000 sign-on bonus.
Requirements: Master's Degree in Social Work, LCSW license, current BLS certification, and preferably one year of supervised clinical experience in a medical setting.
Knowledge: Psychodynamic and crisis theories, brief counseling techniques, and community resources.
Details: Full-time position, $40.00 - $48.10/hour, with benefits including health, dental, vision, life insurance, flexible spending account, and paid time off.
